Upgrades to gear increase its base stats, and sometimes increases its other features. The worth of these upgrades is determined by their tiers, requirements, and the number of random augmentations applied.

Reroll Tokens

Upgrade components can be replaced with new ones, but doing so will damage the previous upgrade, excluding for glyphs which can be removed and replaced at will.
Requirements
Upgrades to items allow players to increase the stats of certain armors and weapons they already have equipped. The requirements for upgrading an item will vary based on the type of item however, generally you'll require Dilithium and a variety of Upgrade Tokens (various options available).
The items can be upgraded by adding item upgrade components to equipment and weapons. This gives them more effects or an improved quality of the item. These upgrades can be obtained from a variety of sources, such as crafting, buying from the Trading Post, karma vendors or looting from bosses and raids. Once an upgrade component is inserted into an item, it cannot be removed and changed without restriction, however, the glyphs used in gathering tools can be swapped out without limitation.
Upgrades can boost the Mark Level or Rarity. Some upgrade components come with additional modifiers that can boost the stats of the item.
For instance, the helm upgrade "Foundation of the Sun" improves base defense and gives an additional 20% chance of blocking. The DPS "Wildfire", on the other hand increases damage to weapons.
Upgrades can be beneficial for certain items, but they are not because they boost stats. They are more valuable for the players when they get the perfect roll. For example, a perfect-rolled 200/6 504 armor set piece is highly valued in the melee PvP community and can be worth 20-50x more than the non-upp'd version.
When you select an upgrade component to slot into an item the game will display a pop-up window that shows the necessary requirements. These include the item type (either weapon or armor) the tier of that item, the Upgrade Token required, and the amount of progress needed to complete the upgrade.
The game will add an item in your inventory after you have completed the prerequisites for an upgrade. The progress bar will update to show you how far to getting the upgrade completed. You can also track the status of an item upgrade by visiting any NPC who upgrades items.
